Fees and Payment
- Self-Pay: My fees are reasonable and competitive for the Chicago area. (Please call for details.)
- Zelle/“QuickPay” accepted and preferred; alternate payment methods arranged upon request.
- All required payments are due at time service is rendered.
Insurance
This practice does not participate in insurance networks. However, you may be eligible for out-of-network benefits from your PPO. You will be provided with the required documentation (“superbill”) which you can submit to your insurance plan in request of any out-of-network reimbursement for which you may be entitled.
